Outreach Diabetes Service is a generic service for all whānau who have diabetes and are in need of support to help manage their diabetes. We provide transport to doctors and hospital appointments. We can advocate on your behalf. We have a diabetes specialist clinic here at Kokiri fortnightly on Fridays for those with Type 2 diabetes. We hold a 6 week diabetes course - Maramatanga in Naenae "By Māori, For Māori" Mā wai tēnei ratonga? Who can access this service?
Our service cover areas are: Hutt Valley, Lower Hutt, Petone, Stokes Valley, Upper Hutt, Wainuiomata and Western Hills. Whakapā mai- Who to contact for more information?
